Our Day Hospice, which is usually based at the main hospice, is currently suspended due to COVID-19. However, our Day Services team are continuing to provide holistic nursing assessment for palliative care patients. They also provide an individualised plan of care which may consist of signposting to other services, telephone support, virtual support or face to face support.
All patients must be registered with a GP within the North Derbyshire CCGs (North Derbyshire and Hardwick) and be referred by a Health Care Professional.

Referrals to Day Hospice are arranged by a health professional involved in your care, for example, your GP, District Nurse or Community Nurse Specialist. We do not accept self-referrals.
Referrals to the Day Hospice will be triaged Monday to Friday. Triage will not take place at weekends.
Referrals to the Day Hospice will be triaged when:
• The referral form has been fully completed and received
• The patient has consented to share records with Ashgate Hospicecare

What happens after a referral is made?
If your referral meets our criteria, a member of the team will contact the patient within 10 working days. They will then complete an assessment by telephone or video call.You can download our referral form here
