The Day Hospice provides specialist palliative care to patients who are living with a life-limiting illness. Patients attend one day a week for a 12-week period. The care is provided within our purpose-built Day Hospice and can provide care for up to 16 patients every Tuesday, Wednesday, Thursday and Friday.
The Day Hospice delivers two programmes of care: the more traditional Day Hospice programme provides medical cover from doctors to help patients with a life-limiting illness manage their symptoms; and the Living Well programme is a nurse-led day which helps patients whose symptoms are more stable.
The nursing staff are led by the Day Hospice Sister and Hospice Matron. The small team are a mix of registered nurses and healthcare assistants and all staff receive specialist palliative care training and education. All the hospice services are available on both programmes, including access to physiotherapy, occupational therapy, complementary therapy and supportive care services.
All patients must be registered with a GP within the North Derbyshire CCGs (North Derbyshire and Hardwick) and be referred by a Health Care Professional.

Referrals to Day Hospice are arranged by a health professional involved in your care, for example, your GP, District Nurse or Community Nurse Specialist. We do not accept self-referrals.
Referrals to the Day Hospice will be triaged Monday to Friday. Triage will not take place at weekends.
Referrals to the Day Hospice will be triaged when:
• The referral form has been fully completed and received
• The patient has consented to share records with Ashgate Hospicecare

What happens after a referral is made?
If your referral meets our criteria, a member of the Day Hospice team will contact you to offer an informal coffee visit.You can download our referral form here
