We provide care and support to people with specialist needs relating to any life-limiting illness to help them stay at home.
Hospice at Home
Our team of Hospice at Home support workers provide direct care to patients within their own homes. Their aim is to provide the highest standard of nursing care for patients and respite care for carers, supporting people in their own home.
They work in partnership with other health and social care organisations to provide care such as bathing, basic nursing and personal care (until satisfactory services can be organised) and offer support with meal preparation for our patients. They also provide respite for carers so they have some time to do the things they need to do and provide emotional support to patients and their families by offering a listening ear and answering any questions they may have.
Our support workers link with the Ashgate Hospicecare Palliative Care Specialist Nurses who oversee a patient’s care package and we are well known by community nursing and social services. We put patients first and offer a holistic approach to support their needs and those of their carers.
The Hospice at Home team compliments but does not replace the responsibility of statutory services.
Community Palliative Care Nurses
Some of our patients decide they would rather be at home (which can include their nursing or residential home). Our Palliative Care Specialist Nurses make this possible by delivering hospice care within the community, including community hospitals.
The team work alongside GPs and community nurses to provide specialist assessment and advice for managing symptoms and ensure patients are able to access the psychological and emotional support they and their family may need.
GPs, hospital teams or community nurses can refer patients to our team. Our nurses may give advice over the phone to health professionals or they may arrange to visit patients to assess their needs and give appropriate specialist advice. All of our patients are given contact details of their Palliative Clinical Nurse Specialist and our nurses can make arrangements to telephone patients for advice and support.
The nurses are based at Ashgate Hospicecare and they work in three teams covering central Chesterfield, North East Derbyshire, and across a large part of the Derbyshire Dales and High Peak area.
Our nurses help people living in these communities access the services Ashgate Hospicecare has to offer and to help them have the best possible quality of life.
Our nurses don’t provide home nursing or night sitting services but can help to arrange this with a patient’s community nursing team if needed.
