The Social Work team are based at the hospice and out in the community. We work alongside other professionals and agencies to understand the needs of patients, their families, friends and carers.
We treat you with dignity and respect and we will involve you to ensure that your choices are at the heart of everything that happens.
We help people to find out and understand what help and support is available if you need it and we can help people to arrange and manage practical support for daily tasks.
We can also make enquiries and arrange safeguards with you, if you are at risk of abuse or neglect. We help you to express what is important to you and to make use of the resources and legal powers that are available for you.
We talk openly about dying and bereavement and help you deal with your worries and think about how you can cope.
We also make sure that the voices of children and young people are heard so that their needs can be met when an important adult in their lives is ill or has died.
